PARENT OR GUARDIAN RELEASE OF INFORMATION

Laws protecting student privacy require that the Disability Resource Center obtain written permission from the student before we can discuss registration, accommodations, or any other information with the student’s parent or guardian. Students wishing to allow the DRC to speak with their parent or guardian can submit this form. Note: The form must be completed by the student and sent to the DRC from the student’s UGA email address.
