Laws protecting student privacy require that Accessibility and Testing obtain written permission from the student before we can discuss registration, accommodations, or any other information with the student’s parent or guardian. Students wishing to allow Accessibility and Testing to speak with their parent or guardian can submit this form. Note: The form must be completed by the student and sent to Accessibility and Testing from the student’s UGA email address.
