In order to process your requests for alternative media, we will need the following information:

- After classes begin, copies of your syllabi
- Four weeks notice
- How to find out your book titles:
- see UGA Schedule of Classes or look on your course schedule for the course name, call number, and professor's name. For title, author, and copyright information contact any of the following sources:
- The University Bookstore

- Call the main number at (706) 542-3171 and ask for Textbooks, OR
- Visit www.efollet.com, select "Georgia," then select "University of Georgia," and then follow the prompts to find out the names of your textbooks, OR
- Visit the University Bookstore and use the computer, check the shelves, or ask for assistance.
- The Department

- Call, email, or visit the department that is offering the class you will be taking. You can find departmental contact information and locations online at www.uga.edu or through UGA Directory Assistance at (706) 542-3000.
- Ask to speak with the person who submits or processes the book orders (usually a secretary) and be prepared to tell them the professor and the name of the course you will be taking.
- The Professor or Instructor

- Call, email, or visit your professor. You may need to schedule an appointment, or call to find out your professor's office hours. You can find contact information and office locations online at www.uga.edu or through UGA Directory Assistance at (706) 542-8719.
Complete the Alternative Text Request Form and submit it:
- Fax the information to (706) 542-7719
- Deliver it to 124 Clark Howell Hall
- Mail it to:
