The Disability Resource Center (DRC) coordinates accommodations for students with disabilities in order to promote equal educational opportunities and an accessible academic, social, and physical environment.
If you have a disability and would like to receive accommodations at the University of Georgia, please complete the Request for Services Form, and send it, along with your disability documentation, to the DRC. Please provide documentation from a licensed professional who has relevant training and experience diagnosing the reported condition. Documentation needs to be on letterhead, typed, dated, signed, and should include the following information:
- The credentials of the evaluator.
- A clear diagnostic statement.
- A description of the methodology used and test scores that support the diagnosis.
- Information on the current functional limitations and prognosis of the condition.
- Recommended accommodations that are related to functional limitations.
Refer to the DRC webpage here for specific documentation requirements for your disability. If you have questions or would like to speak with a Disability Specialist prior to submitting materials, please call the Disability Resource Center at 706-542-8719 or complete a Contact Request.
All materials can be sent or faxed to:
